REGISTRATION FORMFINANCIAL AGREEMENTMEDICAL HISTORYAt the time of service, we ask for payment for the out of pocket costs of treatment, the portion not covered by insurance. We accept your payment in cash, check, or major credit card. We are also happy to work with third party financiers that offer low and no interest payment plans. Both CareCredit and Chase Health Advance financing are available in our office. You can click the link below to get pre-approved for financing from both of these financiers.
